I hereby apply for membership and certify that I have not previously applied to join the International Order of Co-Freemasonry, Le Droit Humain, or any other Masonic Obedience (if otherwise state the circumstances above.  I undertake to continue my membership and to pay my subscription and any outstanding dues to my lodge for three years.  I understand that my details will be stored on the British Federation's database for use within the Order, in accordance with the Data Protection Act guidelines.
